Introduction
The prostate is a key organ in the male reproductive system. With age, it can be the site of a number of pathologies, such as benign prostatic hyperplasia (BPH), prostatitis or prostate cancer.

Role and Function of the Prostate
The prostate is a walnut-sized gland located under the bladder and surrounding the urethra. It plays an essential role in :
✅ Production of seminal fluid, which nourishes and protects sperm.
✅ Control of urinary flow, by influencing the opening and closing of the urethra.
✅ Fertility and sexual function.
📌 With age, the prostate can become enlarged, inflamed or affected by more serious pathologies.
What are the main prostate problems?
1️⃣ Benign prostatic hyperplasia (BPH)
Also known as prostate adenoma, this is an enlargement of the prostate gland that compresses the urethra, leading to urinary problems.
📌 Common symptoms:
- Frequent need to urinate, especially at night (nocturia).
- Weak or interrupted urine flow.
- Difficulty starting or stopping urination.
- Sensation of bladder not being emptied after micturition.
💡 BPH is benign, but can become very troublesome. Fortunately, there are treatments available (medication, laser, surgery).
2️⃣ Prostatitis (Infection or inflammation of the prostate)
Prostatitis is a bacterial infection or inflammation of the prostate, often painful. It can be acute (sudden) or chronic (persistent over several months).
📌 Common symptoms:
- Pain in the lower abdomen, testicles or perineum.
- Burning while urinating.
- Frequent and urgent urination.
- Erectile dysfunction or pain during ejaculation.
💡 Antibiotic or anti-inflammatory treatment is often required.
3️⃣ Prostate Cancer
Prostate cancer is the most common cancer in men. It progresses slowly, often without symptoms at first.
📌 Late symptoms :
- Urinary difficulties (similar to BPH).
- Presence of blood in urine or semen.
- Bone pain when cancer spreads.
💡 Early detection is key to effective treatment. A PSA test (blood test) and digital rectal examination (DRE) are recommended after the age of 50.
What are the warning signs?
🚨 If you experience any of these symptoms, it is important to consult a specialist as soon as possible:
- Persistent difficulty urinating.
- Unexplained pain in the pelvis or lower back.
- Blood in urine or semen.
- Unusual fatigue and unexplained weight loss.
📌 Early diagnosis can prevent complications!
What solutions exist?
1️⃣ Drug treatments (BPH and Prostatitis)
- Alpha-blockers: facilitate urine flow.
- 5-alpha-reductase inhibitors: reduce prostate size.
- Antibiotics and anti-inflammatories: to treat infections and inflammation.
📌 These treatments are suitable for mild to moderate cases.
2️⃣ Non-invasive treatments
If drug treatments are not sufficient, surgery may be proposed:
- Shock waves: used to improve blood circulation and relieve pain.
- Perineal rehabilitation: to strengthen bladder muscles and improve urinary control.
- Dietary and lifestyle changes: reducing caffeine, alcohol and irritating foods.
📌 Indicated as a complement to medical treatments.
3️⃣ Surgery and Advanced Treatments
If conventional treatments are not enough, there are several options:
- Partial prostate removal (TURP, HoLEP, laser, etc.): to relieve BPH symptoms.
- Robot-assisted radical prostatectomy (total removal of the prostate gland): for cancer localized to the prostate.
- Radiotherapy and hormone therapy: specific treatments for advanced prostate cancer.
📌 BPH surgery is considered as a last resort, when other options are no longer sufficient.
